114 Taxpayer Identification Number (TIN)

Policy

Every provider is required to have a Taxpayer Identification Number (TIN). AHCCCS does not accept a Social Security Number (SSN) in place of the TIN. If a provider gets a new TIN, it is handled as a change of ownership. See section 302A Change in Ownership for more information.

Taxpayer Identification Number (TIN)

An identification number used by the Internal Revenue Service (IRS) in the administration of tax laws.

 

Proof

Proof is collected electronically from the Internal Revenue Service (IRS) records. If the name on the AHCCCS Provider Enrollment application does not match the name on the IRS records, a letter from the IRS showing the TIN and name match can be provided.
